HEALTH

Medical Forms
Registered SFS students should update all necessary health forms required by the SFS Health Office, as directed at the end of the school year.
New SFS students who will be attending SFS from Fall 2022, should complete the SFS New Student Health Form in the link provided by the SFS Admissions Office. Please contact the Camp SFS Office if this link needs to be resent.
Returning external students who have a verified Report of Medical Examination Form by the Camp SFS Office, conducted in 2021, will be exempt from submitting an updated health form for Camp SFS 2022.
All other external students of Camp SFS are required to submit the Report of Medical Examination Form within seven days of application. This form must be completed by a licensed physician, that is not a parent of the student, in order for your child to be enrolled in Camp SFS 2022. If any forms submitted are from uncertified clinics, filled out dishonestly, or filled out by their parent who is either certified or uncertified, the clinic may be reported to the the Korean Ministry of Health and Welfare (보건복지부) and/or legal actions may be taken against all offending parties.

In the case that test results are delayed beyond the seven day deadline, the Camp SFS Office may request for the physician’s contact to verify that the examination has been conducted. Depending on the test results, students may be required to receive additional testing if any of their medical forms are not up to Camp SFS health standards.
