Application Checklist
for SFS

Please check for eligibility and review the checklist necessary to complete the online application.

  • Check Eligibility Requirements
  • Non-Refundable Application Processing Fee ($300 US per applicant)
  • Confidential Teacher Recommendation Form (filled out by current teacher and sent directly via email or fax)
  • SFS Early Childhood Assessment (to be arranged upon completed application)
  • All available Report Cards (translated into English and notarized)
  • Birth certificate (for ethnically non-Korean students)
    – OR –
    Korean Family Registry (for ethnically Korean students – original document required)
  • Passport Copies: ____ Father ____ Mother ____ Student
  • Maintain Foreign Passport Form (if at least one parent has non-Korean citizenship)
    – OR –
    Student’s Certificate of Entry and Exit (if both parents have Korean citizenship, including those with multiple citizenships)
  • Online Application Form
  • Check Eligibility Requirements
